Welcome to the SIBS Application!
SIBS is a six week summer program at the UW Madison. Students will learn about the exciting ways that Biostatisticians apply statistics to real world problems by participating in two three credit courses. Seminars, workshops, and field trips are also provided as a complement to classroom training. Tuition, housing and travel expenses are covered by the program. Interested? The UW Madison SIBS will start accepting applications on December 1, 2015. The UW Madison SIBS uses a rolling admissions process, so apply early! All applications should be received no later than March 6, 2016.
The UW SIBS program is open to 15 students. There are no fees or tuition costs associated with participation in the Summer Institute for Training in Biostatistics. Housing, meals and some extracurricular activities are covered. Participants will earn college credits and have access to university computing systems, libraries and other facilities.
Instructions for using the SIBS Application
1. Create an Account
Click on the link “create new account” on the left side of the screen under the 'User login' section.
You must enter the following required information into you profile when first creating an account:
a. Your SIBS Application username (this can be your email)
b. Your primary email address
c. First and Last name
You may complete the rest of your application during your account creation, or go back and finish it later.
The account password and further instructions will be sent to your email address
2. Log In
Log in with the user name and the password sent to you. This is a one time log in, so be sure to RESET your password after first logging in. It is recommended to choose a password that contains at least six characters. It should include numbers, punctuation, and both upper and lowercase letters.
3. Fill in the Blanks
You may complete or update your SIBS application at any time after initially creating your account.
Simply log in, select "SIBS Application" on the main menu, then "Click Here to Access Your Application" at the top of the page. Fill in any missing information, and then select "Save contact" to save your information. You are allowed to edit information once you have formally submitted the application.
4. Contact Your References
Letters from your references will need to be sent separately to Whitney Sweeney. You are responsible for contacting them and ensuring that the letters arrive on time. Letters can be sent either via snail mail to Whitney Sweeney, 4735 MSC, 1300 University Avenue, Madison, WI 53706 or emailed to firstname.lastname@example.org. Emailed letters will need to include some way to identify the source (e.g., sent on letterhead, reference name is included in email, etc.).
5. Order an Official Transcript
To complete your application, you will need to arrange to have one copy of your official transcript sent to Whitney Sweeney, 4735 MSC, 1300 University Avenue, Madison, WI 53706. This is separate from the unofficial transcript(s) that will be uploaded into the online application form.
6. Completing the Checklist
Your application is complete when we have received
a. The completed online application (including statement of purpose and uploaded unofficial transcripts)
b. Two letters of reference
c. An official transcript
