Instructions for Applying to UW Madison SIBS

SIBS is a six week summer program. Students will learn about the exciting ways that Biostatisticians apply statistics to real world problems by participating in two three credit courses. Seminars, workshops, and field trips are also provided as a complement to classroom training. Tuition, housing and travel expenses are covered by the program. Interested? The UW Madison SIBS will start accepting applications on December 1, 2014. The UW Madison SIBS uses a rolling admissions process, so apply early! All applications should be received no later than March 6, 2015.

Instructions for Applying to UW Madison SIBS

1. Create an Account
Just click on the link “create new account” on the left side of the screen under the “Log in” button. Fill in the Account Information and click “Create New Account”. The password and further instructions will be sent to your email address.

2. Log In
Log in with the user name and the password sent to you. This is a one time log in, so be sure to RESET your password. It is recommended to choose a password that contains at least six characters. It should include numbers, punctuation, and both upper and lowercase letters.

3. Fill in the Blanks
Whenever you log in, select "SIBS Application" on the left and "Edit SIBS Application" at the top of the screen. Simply fill in each appropriate blank
and then select "Save" to save your information. If at any time, you decide to go back to a previous section in the application, just click on the
appropriate heading at the top of the screen. You are allowed to edit information once you have formally submitted the application. Click continue at the bottom of each page and click submit at the end.

4. Contact Your References
Letters from your references will need to be sent separately to Whitney Sweeney. You are responsible for contacting them and ensuring that the letters arrive on time. Letters can be sent either via snail mail to Whitney Sweeney, Box 4675, CSC-K6, 600 Highland Ave, Madison, WI 53792 or emailed to Emailed letters will need to include some way to identify the source (e.g., sent on letterhead, reference name is included in email, etc.).

5. Order an Official Transcript
To complete your application, you will need to arrange to have one copy of your official transcript sent to Whitney Sweeney, Box 4675, CSC-K6, 600 Highland Ave, Madison, WI 53792. This is separate from the unofficial transcript(s) that will be uploaded into the online application form.

6. Completing the Checklist
Your application is complete when we have received
1) the completed online application (including statement of purpose and uploaded unofficial transcripts),
2) two letters of reference, and
3) an official transcript.
